Care Planning

A care plan is a documented agreement of a plan of action between the service user and service provider based on SMART* objectives.

Care plans should document and enable review of service user needs, goals and progress across four key domains: drug and alcohol use, health (physical and psychological), legal, social functioning (including housing, employment and relationships)
